New Patient Registration

If you wish to register with us, please complete a registration form which may be obtained from the main reception.

When registering please bring with you

• proof of your address, e.g. a recent household bill or rent book
• ideally a passport, driving licence or other recognised photographic ID.

If you have a non-UK passport please provide proof of a right to register for full NHS services. If you have a Medical Card, please bring this with you as this will help us obtain your previous records.

You are encouraged to see the same doctor or nurse whenever you have a regular appointment. You may also request to see other GPs and nurses if, for example, you need an appointment at short notice or you need to see a female GP.

Where patients are requesting to join the practice list, the practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.

Non-English Speakers

Pre-registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. Also please complete the Online Medical Questionnaire below.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
